Flu Vaccine Consent Forms for Drive Thru Clinics

Flu shots will not be administered at SMSA this year.  In order to continue to encourage families to have their child receive the flu vaccine, the Fond du Lac County Health Department requested SMSA to disseminate flu vaccine consent forms regarding the drive through clinics.

 

Please note, grade P3-5th will have a copy sent home soon in their backpacks.  Grade 6-12 can pick up a copy in the North or South office to take home as well.

COVID-19 Resource Page on Website

Our website now has direct access to the COVID-19 “planning page,” by clicking the COVID-19 button on the top right corner of the homepage! As a reminder, this page houses all COVID-19 related resources, including the “COVID-19 Reporting Form” that you are to use to report if your child(ren) feels ill with COVID-19 symptoms, is being tested, or has been exposed outside of school. This page also houses our weekly dashboard numbers update and the Exposure & Symptoms Flow Chart, which guides families through response protocols if your child(ren) feels sick or has been in close contact with a confirmed case.

 

The pandemic is a fluid situation, so as information changes, we will regularly update this page and encourage you to check back for updates.

Food Service for Virtually Learning Students

Food service is available to families who are educating virtually at this time. Families can submit requests, each week, by clicking here.

 

Please submit your selection, each week, no later than Fridays at 5:00 p.m. so we can process the meals for pickup. On Monday afternoon, between 3:30 to 4:00 p.m. after the buses have left the loading zone, you may drive up to the loading zone/dock area next to Baker Gym and call food service at 920-322-8075 to let them know who you are picking up meals for and they will deliver the meals to the vehicle. You can reach out to Mary Karrmann at mkarrmann@smsacademy.org or 920 322-8075 if you have any questions or need to arrange a different pickup time.

 

Caution Food Safety:

Because of perishable items, please make sure your meals are either consumed or refrigerated soon after pick-up.

2020 Soles for Catholic Education Walk!

Catholic school students, parents, teachers, alumni and friends from across the archdiocese will lace up their shoes to participate on Saturday, October 17th, for the 2020 Virtual Soles for Catholic Education Walk.

Final Call for Clipped Box Tops - Last Day to Bring in October 9th

During this challenging time, our teachers and students need more help than ever. No matter what school looks like, Box Tops for Education is a fast and easy way for you to help out from home.  

 

This is a request for ONE FINAL COLLECTION of clipped Box Tops and to encourage everyone to use the Box Tops app.  The collection form is attached — attach your clipped Box Tops to the sheet — please check to make sure the Box Top has not expired — and bring them back to your home room teacher by Friday, October 9, 2020.  Click here to download the collection form.

 

The Box Tops program has been around for over 20 years, and it really works. In fact, Box Tops for Education has helped give almost a billion dollars to schools since 1996. Over the years SMSA has participated we have received over $52,420! Last year with the clipped Box Tops and families using the Box Tops app we earned $1,442.  

 

The Transition from Clipped to Scanned Box Tops.  In 2019, the program evolved to become digital so that you can earn for your school with just the touch of a button.

 

HERE’S HOW IT WORKS: Buy participating products and use the Box Tops mobile app to scan your store receipt. The app identifies eligible products and automatically adds Box Tops to your school’s earnings online. Even if you’re shopping online or doing grocery pickup, you can still submit your e-receipts and earn Box Tops. Twice a year, our school gets a check!

 

Your earnings may seem small, but they really do add up. The more people that participate, the bigger the impact we can make. So let’s give our teachers the support they need and help give more kids the opportunity to dream. Little by little, we can help make a big difference.
